Number broken: One (Haven't replaced it yet)
Age of car: Less than a month (when it broke)

When driving, or "it just appeared": It just appeared, although I noticed it after a drive down to Exeter. Didn't hear it go or anything.
Cost of replacement:

It didn't crack, more shattered but stayed in tact, until the drive home when it completely went (again I didn't hear it happen, just when I got home the glass was gone except for a few small shards)
